Dog License Renewal Form for Mail-in Registrations
Fees and Information:
| State Law requires dogs, 6 months or older, to be licensed, and then every year after that in June. |
| Dog License renewal begins every year on June 1st. |
| Provide updated rabies certificate and spay/neuter certificate if this information has changed from previous year. This information is also needed for newly adopted dogs. |
| Rabies date, per law, must be July 1st or later for current licensing year. |
| To avoid late fees, license by June 30th. |
